SPORTS ACADEMY OF IDAHO, INC. DBA TEAM GENESIS BASKETBALL WAIVER and RELEASE

Notice to Parent or Guardian: Do not sign this agreement before you read it. I acknowledge that, by allowing my child to participate in Sports Academy of Idaho, INC DBA Team Genesis Basketball activities, I agree to the terms of the this waiver, as set forth below.I would like my child to participate in Sports Academy of Idaho, INC DBA Team Genesis Basketball  . 
As lawful consideration for being permitted to participate in the Sports Academy of Idaho, INC DBA Team Genesis Basketball program I agree that I will not make a claim against, sue, attach the property of or prosecute Sports Academy of Idaho, INC DBA Team Genesis Basketball and their agents, sponsors and employees for damages for death, personal injury or property damage which my son/daughter may sustain as a result of my son/daughter’s participation in these sporting activities. 
This release is intended to discharge in advance Sports Academy of Idaho, INC DBA Team Genesis Basketball and their agents, sponsors and employees from and against any and all liability, claim, loss, damage, cost, or expense for negligent actions, or attributable in any way to, any negligent act or omission on the part of any such person or the Sports Academy of Idaho, INC DBA Team Genesis Basketball arising out of or connected in any way with my son/daughter’s participation in the basketball club, camp or clinic. I further understand that sports involve physical contact between players, that serious accidents occasionally occur during such sporting activities, and that participants in such sporting activities occasionally sustain serious personal injuries (including death) and/or property damage, as a consequence thereof, knowing the risks of participation, nevertheless, I hereby agree to assume those risks on behalf of my son/daughter and release and hold harmless Tree of Hope/Sports Academy of Idaho and their agents, sponsors, and employees who (though negligence or carelessness) might otherwise be liable to me (or my heirs or assigns) for damages.  
1. Information. This is a legal document that releases liability from Sports Academy of Idaho, Inc. all D.B.A’s including, but not limited to, Team Genesis Basketball, 5 Level Scoring Camps, and has the undersigned waive certain legal rights regarding contracting any types of injuries, abuses, viruses, diseases, and/or illnesses in basketball competition associated with Sports Academy of Idaho, Inc., and all association there with. 
2. Releasees. Releasees, is defined to mean all individuals or entities, directly or indirectly providing any services associated with Sports Academy of Idaho, Inc., which the undersigned is or may become involved with during all activities on behalf of Sports Academy of Idaho, Inc. Releasees should include, but is not be limited to, board members, coaches, and anyone associated with or on behalf of Sports Academy of Idaho, Inc., and the independent contractors, staff and assistants of Sports Academy of Idaho, Inc.
3. Agreement. This agreement to hold Releasees harmless is for the express purpose of benefiting each Releasee through Sports Academy of Idaho, Inc. Revisions here of may been forced by any Releasee. The Waiver Agreement is absolute, unconditional and cannot be rescinded and is independent of any other contract involving the undersigned and applies to undersigned’s heirs, legal representatives, successors, assigns and anyone else claiming under them personally or on behalf of the minor child. To the extent the terms of this waiver agreement modify or conflict with any provisions of any other signed agreement or contract involving the undersigned, the terms of this release and waiver shall control. 

I have carefully read this agreement and fully understand its contents. I am aware that this is a release of liability, and that I am waiving any and all rights that I may have to bring a lawsuit in which I could assert a claim on behalf of my child against Sports Academy of Idaho, INC DBA Team Genesis Basketball and all the other persons mentioned above for any injuries or damages caused by the negligence of the aforementioned parties. 
I also acknowledge this is a contract between myself and Sports Academy of Idaho, INC DBA Team Genesis Basketball and their agents, sponsors and employees, and I have signed it on my own free will.
